Your tension is incorrect, either the top tension is too tight or the bottom tension is too loose. Tension is a delicate tug of war between top and bottom thread to get the hooked loop of thread dead centre in the pieces of material. The is also a slight possibility that you are using a need that is too large and making a hole that is too big for the thread, and is making it loose. Loops on top of the fabric mean the top thread tension is too tight. Decrease tension (lower number) with small increases at a time.

Threading and setting the tension on your Finesse 356 sewing machine can be done by following these steps:
Raise the presser foot and thread the machine:
Take the end of the thread from the spool and pass it through the tension disk at the top of the machine.
Next, guide the thread through the take-up lever and then down through the thread guides on the front of the machine.
Pull the thread through the needle from left to right and make sure it is securely in the needle's eye.
Set the tension:
The tension dial is usually located on the top or the side of the machine, near the needle.
To set the tension, turn the dial until the numbers on the tension gauge line up with the red indicator line.
The tension should be set to a medium setting, around 4 or 5, but you may need to adjust it based on the type of fabric you are using.
Test the tension:
To test the tension, sew a few stitches on a scrap piece of fabric, and then examine the underside of the fabric.

Usually, there are more than one or two pieces printed on a pattern sheet. Look around some more. Also, check that the same pattern piece isn't used for more than one fabric piece.
Your pattern instruction sheet should have a diagram of the pattern pieces, if you dont have this, then the pattern pieces should have their name on them. Wow, I finally get to have a solution. 1. Turn the machine off. 2. Remove the hoop. 3. CAREFULLY slide the bar on the embroidery unit to the left. 4. CAREFULLY slide the piece where the hoop attaches toward you. 5. Turn on the sewing machine. The machine should take the unit to "standby" with a click. 6. Attach the hoop with the power on. 7. Exit the Futura program. 8. Restart the software. This "should" fix the problem, however, you may have to repeat this process more than once to get it to "stick".
