The answer will depend a bit on what sort of phone you have.
If you're on an iPhone or iPad, you generally hold your finger down on the icon for the app for a couple of seconds, and all the icons on the screen will start to jiggle a bit, and each will have a small 'X' near their top. If you then touch the 'X' on the icon of the app you want to get rid of, it'll be deleted from the phone. (There are some 'native' ones from Apple that they won't let you delete without doing a lot more work).
