Adjust the audio output on your playstation and the television depending upon how you're connecting it to the television for picture and sound.

For an hdmi cable between the television and the PS3 only then adjust the audio ouput to hdmi on the PS3.
You do not need to connect anything else between the television and the PS3 besides an hdmi cable.

If you are using the colored plug type component or composite cables then it is av.

If you are connecting with a fiber optic cable to a surround sound system then it is set to optical.

Depending on what television you are using you would also have to change the Playstation to 48khz/96khz

You willneed to have this unit serviced I'm afraid. You have a laser assy problem, or one of three servo circuits are not working. That would be the focus, tracking or the spindle motor circuits. Good Luck
