There are 3 methods to format the phone

First: if the phone menus work there is a; go to menu -> tools -> settings -> general -> factory restore. note: the defualt code is 12345
Seconed method requires you to enter these into the dial number area. Nokia Code : *#7370# : NOKIA Format phone (Soft Reset)
Nokia Code : *#7780# : NOKIA Factory Reset (Hard Reset)
Found these in the N95USERS fourm. Note: the defualt code is 12345


Third: is a button combination the needs to be done during the phone powering on
Hold down. 3, * and Green Call button While holding this combination press the power button once to start the phone Note: this does not require any codes; if this works the phone will prompt you to be set up again

None of these methods format the memory card

There are two methods to format the memory card
First: Go to menu -> tools -> utillites -> memory -> options -> format mass storage
Seconed: Connect your phone to your PC via the supplied USB cable. Then select Mass Storage mode on the phone. Once you PC has installed the Mass Storage device you can format the card from My Computer. Note: assuming Microsoft Windows XP or VISTA.

type *#7370# into the phone while on. Remember to back-up your data using the Nokia PC Suite. Also make sure you remove your memory card if you have one, and remove the password from the memory card before you try to reformat.

to format nokia N95 use :
Soft reset code *#7370#
Hard reset code *#7780#

* + 3 + Calling key + On/Off key till formatting appear, Other wise flash ur phone

*=Turn off the mobile device. Then press and hold “3", the “Green button” and “*” - Turn on and keep holding the keys untill the mobile phone restarts and prompts you to enter details.

You can make free phone calls with GA628 (also work on other models)

NOTE: This trick has only been reported working on PREPAID GSM CARDS and in some countries and with some software versions.
The prepaid GSM SIM CARD is a kind of "SIM card" which only has a certain amount of credit on it.
Here's the trick: Dial the number normally and press YES. While "connecting" is shown on the screen, the following procedure should be carried out:
Press CLR then 0 then # and then NO (twice) so as to switch OFF the phone. You can then still speak on the phone while it is switched off but the SIM card does not record your calls which will lead to FREE phone calls in some countries.. I hope!!
