It must be a G3 or G4 iPad. The sim is locked and you need a PUK. The puk was on a card with the sim that went in, together with the initial pin. When someone locked you phone to tablet, by entering the pin (for the sim) wrong, 3 times in a row, you can unlock it with the PUK.
If you can't find the PUK, goto a store of your provider. Sometimes they can give you the PUK and if not, they can give you a new sim, with a new pin and puk, and put your number on the sim.

As I understand from personal experience, the PUK code is only available from the service provider. You need PUK code after you entered the wrong PIN 3 times for the SIM card. Call your service provider or get a new SIM card.
