Those are a sealed unit. usually once you open them up they won't reseal well and you will replace cloudiness for moisture build up. Most common to replace the whole headlight assembly as a unit. Ebay motors is a good place to find inexpensive headlight assemblies.

if you can get the clear part separated from the backing you can polish this clean. It takes a little bit of skill and concentration and patients but it is possible

If there fogged from the inside you'll need new headlamp assemblies. If there fogged on the outside they can be restored. I do headlamp restorations all the time. The trick is to know if the problem is on the inside or outside. Also if the "fogging" is condensation than for sure they will need to be replaced.
