SOURCE: puk blocked!
Here's what it means:
PIN Unblocking Key (PUK) - If a GSM or GAIT subscriber enters the wrong PIN three times in a row, then the wireless mobile phone is locked until the correct PUK is entered.
What to do:
Call ATT and ask for the PUK code. You will need to verify the account when you call, so if you do have the info (last 4 of the SSN on the acct or the password)
Other option is to bring that to a cellphone repair shop and have it hack on its PUK .
Hope this helps... Don't forget your rating.

Most SIM cards are blocked at the time of purchase. If your SIM card lock is on you have to enter your PIN (Personal Identity Card) every time you turn on the phone.
If you enter the wrong password three times in a row the SIM card gets blocked. This is indicated by the message PIN Blocked To unblock this you need to enter your PUK
card (Personal Unblocking Key). Your PIN and PUK is supplied by your service provider. You can edit your PIN and select a new four to eight digit number
