You can just go to your app store, download snapchat. Register on the snapchat app with your mobile device and the you are in. Snapchat only works on compatible mobile devices.
I don't use snapchat, but would think it works the same as just about every other program and app of that sort.
General rule is there is a "forgotten password or username" link somewhere on the login page or sites web homepage. As long as you can remember the email address you used at sign up and maybe a couple of security questions, they will email you a link with new details.
Failing all that, just re-register although you may need a different email address as the original will come up as already in use.
