Question about Coffee Makers & Espresso Machines
It depends on the make and model of the machine but this sounds like the one way valve in the pump is requiring more pressure for it open so the pump has to work a bit harder to push the water through resulting in the pulsing you experience.
allow the machine to cool, purge the steam and hot water then remove the pump assy.
Examine the NRV for free movement and check its operation. It may be scale causing it not to open properly.

definitely clean out the sprayhead. also, you may need to reset the dispense time on the board behind the front panel. the instructions are on the panel itself. make sure the switch is on "lock" after you are done.
