PUK code is obtained from your service provider for free!

The PUK is a PIN unlocking key. This unlocks the SIM card so that it works with your device. This SIM lock can be activated if you fail to enter your PIN correctly several times. It can also be remotely activated if the phone has been reported lost or stolen. You need to obtain the PUK code from your cell provider since it will be specific to your device/phone number. It may be on the SIM certificate from when the phone was activated. If you mistype this number (8 digits long), the SIM card may become permanently locked and need to be replaced.
For AT&T wireless, go here: http://www.att.com/esupport/article.jsp?sid=33534&cv=820#fbid=2HamKjHOrxl to find the directions for obtaining and entering the PUK code on the phone. Links to other wireless phone providers' sites to obtain a PUK or PUC (PIN unlocking code) are available here: http://www.geeksquad.co.uk/articles/Mobiles/2012/04/how_to_find_a_mobile_puk_code .
I hope this helps.
