I have a deluxe precision sewing machine and the bobbin and holder keeps coming out

Perhaps the latch on the bobbin case is malfunctioning. You could probably try a new bobbin case. If that doesn't work, then something in the machine itself where the bobbin case latches may be worn.

Why has it stopped working?

Most of the time if the bobbin won't engage is you are not making a connection as its has a certain way it goes on. Most New Homes are proprietary on how the different parts engage. If you look and not think too hard take your time, you will discover the proper way to get the bobbin to engage.

The bobbin winder does not spin. it does turn manually, but not when I step on the presser foot. I took the top piece off and see no bobbin winder "tire" - should there be a "tire"

You should have a bobbin winder rubber it has probably come off and fallen in the bottom of the mahine, they are a standard size and an be bought from any sewing machine shop
