Mine was doing that for a while, finally found a stray bit of thread in the tensioning disks. Hard to find or remove... took a small flashlight to see it and teased it out.

The needle thread breaks
1. the needle thread is not threaded properly
2. The needle thread tension is to tight
3. The needle is bent or blunt
4. The needle is incorrectly inserted
5. The needle thread & the bobbin thread are not set under the presser foot when starting to sew
6. the threads were not drawn to the rear after sewing
7. The thread is either to heavy or to fine for the needle

You might have an old needle. Most sewing experts recommend changing the needle between each project. As the needle ages the eye of the needle becomes rough and cuts the thread.
Alternatively, you might be using the wrong needle. Depending on the thickness of your fabric and the type of thread you are using you should adjust your needle type. There are needles for denim and needles for metallic threads.
When I have chronic problems like this I rethread both my upper and lower thread and that usually resolves the problem.

When threading machine, thread won't stay in place at step 4, so when I move thread to step 5, thread comes away. Only bought mchine in March.

When threading the upper thread, be sure to always raise the presser foot. This releases the tension disk and allows the thread to seat properly.

The upper thread tension keeps breaking the thread, no matter how low I set the tension. How do I fix it?

Not a technician, but I had same issue not long ago, it was old dried out thread. Once I replaced with new thread, no more issues. Something easy to try. Also might want to double check to make sure no thread is between the pressure plates. I use canned air to spray them out once in a while between annual cleaning.

I have a Quantum CXL Singer sewing machine and the thread keeps breaking. It will make about 3 stitches and breaks. It is breaking in the same place.

Make sure the thread coming off the spool is not wrapping around the spool spindle. Install a brand new needle. Remove ALL the thread from the machine. Clean bobbin area of any dust, lint, threads. Set upper tension to the midpoint. RAISE the presser foot and then totally re-thread the machine from beginning to end verifying the sequence is correct. Test.

***Upper tension setting
*** Put the pressure foot up.
***As you are threading give little tugs to check for snags. Snags must be fixed.
***When finished threading (not the needle) put the pressure foot down.
***Check the tension by starting at zero and increase the tension slowly and give small tugs to see if the tension is rising. (This is all done without the needle being threaded)
***When you feel comfortable this is working then thread the needle and set the tension at four.
***Set the bobbin tension. Attachment gives instructions.
Now that all of the tension has been set you can try for

My singer 8500q keeps coming up with a 06 error (top thread breaking) but it's not. Also tension not working or adjusting.

Probably a broken or stuck check spring (which is also part of the upper thread sensor). Also could be lint the tension disks if the thread is pulling down to the bottom.

Upper thread breaking on singer machine

Check that your bobbin thread is not wound too tightly or too loose. Also adjust your tension down a click at a time until it stops breaking, testing on scrap fabric. Singer provided manuals online. Look it up here: http://www.singerco.com/accessories/manuals.html
