SOURCE: micro SD card
try to format it using your computer.
Connect the card with computer using a card reader
Then go to my computer right-click the micro sd-card and select format. Format it using FAT.

i found something to fix it. or at least it worked for me
Install that, then run the program and CAREFULLY select your card from the formatting list
then do a low level format
Then format it on windows or phone or whatever
PLEASE BE CAREFULL NOT TO SELECT YOUR HARD DISK IN THE PROGRAM. IT WILL VAPORIZE ALL YOUR DADA

I had the same problem and I tried everything. I couldn't think of anything else to do so I just turned my phone off and back on again. When I went back to the memory card menu on my phone all the folders that I lost re-appeared. I don't know if it would work on other phones but it's worth a try!
