Yup I fear your battery is gone faulty.. A common thing. You will need to obtain another. as an aside here try to say charge your phone up overnight, everynight, and use it during the day, also try to do it for at least the same amount of time each time you do. This will help with battery life.

it realy soun sounds like liquid damage, take it in for repairs, if you wnat to try youreself open the phone wash it with metholated sprirts and see you should be able to see the damage white powder like
