Tablespoon is a unit of volume, milligram is a unit of mass (or weight). The conversion depends on the density of the substance.

850 milligrams of water is about 0.0575 tablespoons. 850 milligrams of mercury is about 0.00425 tablespoons.

What is scant in baking

Let's us the example; scant tablespoon. Heaping tablespoon would be to use a tablespoon, but heap on as much as the tablespoon will hold. Well, a scant tablespoon would be a tablespoon measured with absolutely no overflow at all, or even a very small indent in it.

How many grams in a tablespoon

There is no direct conversion between the two. Gram is a measure of mass while tablespoon is a measure of volume. For example, a tablespoon of water will mass about 15 grams while a tablespoon of gold will mass about 200 grams.
