Firstly i think you should call your mobile phone service provider. If you google them you will find a landline number for them.
They should, i believe be able to give you the code needed to open the phone to be able to use it again.

They will need a few details from you and will ask if you have enterered the wrong code 3 times and is displaying please enter PUK code.

This is the easiest way around your problem i believe.
