Try going into the menu of the camera and reset your camera. Other than that you may have a bad lens.

Your problem is that you have selected a setting for a conversion lens (an add on wide or tele lens)!
You'll find the setting under REC 3/3
menu - up - conversion - OFF
Drove me nuts for a while!
