I`ll have to be the bearer of bad news, the image is not burned onto the screen it is burned into all 3 of your picture tubes, which basically means the set is junk because the cost is too high.

unplug both the TV and the cable box for 2 minutes then replug them in. this will reset the micros in both units. if there is still no sound check your SAP setting make sure its off.

Yes, the lamp will gradually get darker as it nears it's end. The rolling lines sounds more like the set is on a local channel for the cable hook up. If your local area has a channel 3 or 4 live terestrial broadcast simply tune the cable box output to the other one, the same for the tv.
If this is cable/local broadcast issue it will go away in Febuary when the NTSC broadcast ends.

My exact same problem turned out to be the cable box not the TV. Try unplugging cable box for 5 minutes then power up! Solved my problem!
