Start with the soft reset code: *#7370#
If that doesn't work, try the hard reset: *#7780#
Finally, try the complete reformat:
Turn the phone off, take the battery out, put it back in and hold down "3" "*" and the Green phone button, press power and keep the keys held down until the phone starts asking the setup questions again.
