A PUK lock means you entered your passcode incorrect too many times. The ONLY fix for this is to:
Either call your provider and ask them for the PUK code, OR
Go in to a store from your provider and ask for a new SIM card.
If you enter 3 incorrect PUK code, there is no way you could save your SIM.
