Please change the setting print direction from bidirection to unidirection.
How to check print head Epson lX300+
I tried replacing the motors and the issue persisted, so I have to assume it was a board issue. Switching the printing to unidirectional as suggested by Albert124 took care of issue for us, as we were not concerned about printing speed, since our equipment runs at 300 baud.
The problem is with printer's carriage unit, just replace and check it.

qlenfg We use the LX300, LX300+ and LX300+ II printers with some of the equipment we sell and service. I thought I had seen every problem on these models until I had a unit printing double on the NLQ fonts.
Doing a test print, the characters look fine for the first couple of lines, then the letters start slurring. It appears the two passes required for the NLQ fonts are not aligned -- the alignment gets gradually worse and worse with each line printed.
I have cleaned and lubricated the guide rod the print head travels on, which is usually an issue with the units we see, but the issue remains. I'm guessing the issue is either with the main board, the motor that rolls the platten or the motor that moves the head.
I have a spare chassis with working motors I will try first. Unfortunately, I do not have any spare main boards.
