I'm thinking it could be the ball bearing & spring but cant find any instructions on how to replace it.
I have a 1996 Tacoma that I bought new. In the intervening time, I've put close to 220,000 miles on it. Around 4-5 years ago, 1st and second gear began popping out especially during warm-up. Fearing that the repair for the problem might be extremely costly, I put off the inevitable for several years.The problem grew worse until I found I was holding 3 of the 5 gears in place. After some research, I discovered that the problem is very common on these trucks and to my surprise, the fix was not only inexpensive ( $20) but easy.
In order to perform this repair, you will need a Philips head screw driver and some general purpose grease. The job takes 30 minutes. You will also need to order parts from a Toyota dealer or equivalent. The parts you need are: 1 shift lever seat and 1 shift lever cap. Bring your VIN number as there were slight variations.That's it! So here it goes.

Check that the gear lever is not restricted in the movement and that the pivot pin in the tower for the lever is tight as this will not allow full engagement of the gear. Other reasons for gear disengagement are misalignment of the bell housing to rear of the motor, failing clutch shaft bearing, failing spigot needle roller bearings between the input shaft and the main shaft, tapered lugs on the syncro dog and gears , and worn extension housing bush or if it has a transfer case the a failing bearing between the gear box and transfer case.

I've been resetting pajero's compasses but try to get to an area where you can do 360 degrees turns (empty car parks, ) press the reset button and make 2 complete 360 turns slowly and normally this should set the compass bearing.

Refer to the diagram and torque bolts in sequence as shown in 3 passes. Bolt thread should be lightly oiled. Bolt holes clean of dirt.
1st pass : 39 N.m. (29ft lb)
Make a mark on the bolt with white-out/chalk or similar in the forward facing edge of the bolt.(see diagram)
2nd pass : turn bolt through 90 degrees
3rd pass : turn bolt through a further 90 degrees.
Using a mark on the bolt makes it easier to keep track of where in the sequence you are.
