The singer 221 needs to be threaded with the flat side of the needle on the left side. The thread passes through the eye of the needle from right to left. If this does not solve your problem it may be out of time. You can remove the needle plate and watch the hook it should pass above the eye of the needle as you turn the handwheel towards you. First you need to make sure the needle is at the correct height before adjusting the timing on the lower shaft. You may want to take this in for service if your timing is off.

pull the bobbin case out by pulling tab or flapper forward (outward toward you) and take bobbin out of bobbin case, If bobbin has thread and that it is coming off bobbin free and is not hung up on bobbin case.

I just had this problem, too - if you have the manual, check out page 15. If you have removed the throat plate, you need to make sure that the position finger on the bobbin case base enters a notch on the underside of the throat plate. Fixed my problem right away.
