This is your government passport department's job. You need to contact the passport office in your city or your country's consulate if you are in another country to apply for a replacement passport. I suggest you phone for an appointment and ask them what documents you need to take with you when you go for your appointment. You can also ask for WEB site URL so you can apply to renew your passport and get details of what documentation you need to submit with the renewal application.
