First off, it's a left-handed thread. I wedged the plastic part with a screwdriver to stop it from moving. Get a socket that fits the hex head and turn (counter-clockwise when facing the bolt head). It came off pretty easy. The problem is a replacement, I couldn't find it. I used 1300l glue to re-attach. I then took a hot glue gun and filled the seam between the plastic pad and foam (the entire circumference). Any industrial glue from a hardware store may work. I must admit, I haven't tried the fix yet, although it seems pretty solid. That's how they are attached at the factory, clear glue.
