On your JVC wiring harness the speaker wire colors are:
Left front (+) - White
Left front (-) - White/Black
Right front (+) - Gray
Right front (-) - Gray/Black
Left rear (+) - Green
Left rear (-) - Green/Black
Right rear (+) - Purple
Right rear (-) - Purple/Black
You can download a the manual from Crutchfield. A complete wiring diagram with color codes is on page 41.

sounds like you do not have the yellow wire connected to a constant power source. you need to connect the yellow wire to a source that is still hot when the key is switched off.

For the numerical code given, enter it using the am/fm presets buttons. Please note, if you try too many times the radio will lock up for a certain amount of time.
