The frother gradually gets milk buildup and loses suction- an air blast through the milk tube and up the spout (can get an aerosol at hardware store) will help as well as the cleaning cycle. Also I used a q tip to the top of the spout several times but just had the q tip top come off and may have blocked it completely

Sounds like the venturi tube could be blocked. My model is a CVA 4085
and I can take the assembly out that sucks in the milk, mixes the air
and stem and spits out frothed milk. If yours is similar, see if you can
can reach the top of the milk outlet tube and rotate the ring around it
anti-clockwise (if you were looking up from the bottom) and take the
whole assembly out. If you have never taken this out, disassembled it
and rinsed everything out thoroughly under a running tap now is a good
time to do it. Then check the venturi tube: the clear (ish) bendy
plastic tube that that points up and has a cone with a hole in the top.
If that hole is blocked you won't get very good froth. If you can't
clear it with warm running water then VERY CAREFULLY use a small needle
to push through the hole at the top, then put everything back together.
Brand of milk definitely matters... Here in Canada I used Sealtest 1% when I first used it and the froth was thick light and luxurious. Then one week picked up Neilsen 1% and found it produced absolutely ZERO froth. Back to Sealtest...back to froth. Milk brand matters....but don't know why.
We bought a Nespresso Milk Frother in 2010 for $100. The frother is the same one that comes with the Citiz machine but is a stand alone model. It froths soy milk very well. The less soy you put in it, the more froth you get but even if we fill it to the max line we still get a reasonable amount of foam. We use Silk brand soy milk. I have no idea if foaming is impacted by atmospheric pressure but it may be a factor to consider if you are using all the same factors as us but not getting great results. We live at a higher elevation Calgary, Canada in the foothills of the Rocky Mountains.
Our aeroccino worked fine for 2 years and suddenly it didn't made foam anymore. We tried the "inverting trick" without much success. We contacted Nespresso and they suggested switching from regular fresh milk (we used 2%) to UHT milk (harder to find in US) and to use it cold and freshly opened. So we tried that and it works. Hope this solution works for you too
I purchased a unit second hand for my wife's birthday today. It's only 4 months old, got it home, tested it, same problem... No froth!
I've read all the posts here and all were very helpful troubleshooting guides however they did not solve the issue.
Understanding how the frothing mechanism functions is half the battle in figuring out the solution. It's a simple vacuum type system. No doubt you've read about checking the orings. This is definately the first thing to check however if this is not the reason then consider the following.
The larger of the white tubes that has the red and black oring inserts deep into its port on the tap. Why? It pushes steam past the milk pipe directly into the chrome frothing tube which causes a vacuum essentially drawing milk up it's hose creating the froth. What you can't see without further investigation is that on the underside of the tube centered between the red and black orings is a small hole. This hole is responsible for drawing the milk upwards. If this is blocked, and it probably is then no milk and no froth. Use a really small screwdriver to carefully clean the white tube both from the front and the little hole underneath.
So in conclusion. Any air leak between the red and black orings will prevent the vacuum required to draw milk up the tube and alternatively any blockage of the milk channel will also obviously prevent the flow into the frothing mechanism. Hope this helps.

Remove the black plastic outer sleeve of the Frothing Wand (some models have a metal outer sleeve). If done correctly, the “sleeve” has been removed, but the inner wand with red or black washer, is still connected to the machine. 2) The part of the sleeve assembly that you have removed is comprised of 2 pieces, the frothing sleeve and the center piece. The center piece can be seen if you look into the top of the wand. 3) Using a pencil (eraser end), or similar shaped object, push down on the center piece until an audible “click” is heard. The “click” means that the centerpiece is back into the proper position. When in the proper position, the center piece visibly sticks out the bottom of the sleeve about ¼-inch. It looks like a bird beak. 4) Now put the sleeve back on the machine and your machine should be ready to froth again.

Does this happen after you make several froths or the first time you start the machine? Try removing the panarello and froth milk that way then see if this is happening too without the panarello. I found that panarellos are great as a "quick learning to froth?- but they do have the tendency to froth milk too much without heating it up ( to the temperature that you like).
