Check the heater cores. There are usually 2 in a T&C. Also check the pipes running to the rear heater core. I recently had to have mine repaired because they were leaking!
the thermostat is place between the engine block and the radiator either in a housing on the block or in the top radiator hose , follow this u-tube link for step by step guide , nice and easy Thermostat replacement on 2002 Town and Country

the relay is located in the power distribution box under the hood the relay will be a square black or gray cube just pull out and make sure you rerplace it with the same type relay. refer to your owners manual for any questions regarding the fuses and relays
