It will depend on the insurance you purchased. If it was a 3rd party insurance then you will contact the 3rd party insurance company. You can find out on the documentation for the plan or at the place where you purchased it. Sometimes what people think is insurance for their Apple products is actually the Applecare Protection Plan. If this is the case you need to find out which one. If it is the basic Applecare then it will not cover accidental damage. But Apple offers out of warranty service for a fee. Usually on iPad it is roughly half the cost of a new one. If it was the Applecare + then you'll contact them. Typically, out of warranty replacements with this plan are $49 plus tax & you get up to 2 of them.
