check the setting of FRONT MIC/EXT MIC knob or REAR MIC/LINE knob might be too high,

lower it a little

check also input signal level may be too high, try lowering it

You need to verify that the signal going IN to the recorder is ok. There are a number of ways you can do this, the simplest and cheapest is to get another recording device you know works, that takes the same connectors, and test using that. Something like a cassette deck, or whatever a church member may have at home. If that works, you've determined that its the Tascam, if not, then you need to trace back to see where the signal failure is occurring.
