Depending on how jammed it is and how much you care about salvaging the card, have you tried pulling it out with very fine needle-nose pliers?
You may want to secure your phone and use both hands with fine pointed tools to remove the card.
If this appears to be too much trouble or you cannot get it removed, you may need to have it serviced.
