The first day of summer vaca we surmized our kid had swiped the password to add time to his xbox. We went to create a new password, but when we click on the ''console controls'' button nothing happens. It's like that function has been disabled entirely.

Basically, in order to prevent smart kids from finding a default reset code and disabling the parental controls without their parent's knowing, each Xbox console has an unique reset code that is calculated from the console's serial number.
So if you are unable to remember your secret question to reset the console yourself, you will have to contact the official Xbox support here:
When you are talking with a support agent, you need to confirm, that you are an adult, and then give them the console serial number.
They will then walk you through the process of unlocking your console.
Go in http://www.xbox.com/en-US/live click on sign in, then forgot my password.

I'm not an XBox user but my sons are and I think we're helped them out with this a couple of times. If I remember correctly, we went to the XBox website (using a computer, not the XBox) and reset it there. If we did not have the password to log into the account, we clicked "Forgot Password" and a reset email was sent. We used that email to set a new password and then the boys went and used that new password on the XBox without any trouble. Again, it has been a while, so some of the details might be slightly off but the general concept - go to the website and reset there - I'm fairly sure about. I think it's also possible to do it over the phone with Microsoft support if you have time to wait on hold for a while.
If referring to Xbox Live Gold Family Account, then contact Xbox support directly. You will need to be able to positively identify yourself as the original owner of the account. In other words, no kids or teens posing as adults to gain access to the account.

Here is the procedure to reset the parental lock code:
From the XBOX dashboard screen, go to settings and then parental control.
When prompted for the password, perform the following sequence using the XBOX controller: X, Y, Left Trigger, X.
Enter in a new password, or disable parental control entirely.

Note If multiple controllers are connected to your console, make sure that you use the player one controller. You can recognize the player one controller because the upper-left quadrant of the Guide button is lit up on that controller.

While the console is starting, press and hold the Y button and pull the right trigger at the same time. Continue holding the buttons when you see the Xbox 360 logo.

The console will restart and you will see the Xbox 360 logo again.
The console's display settings are now reset to the defaults.
