You will need to know the IP address of your PS4, so either set it up manually or if your router has the ability, check what IP the router handed out to the PS4 automatically. It is highly advised that you setup the IP address manually, so it doesn't change on you one day, making everything you do worthless.
Ok the easiest but potentially the most dangerous, because it exposes the console to the internet without the protection of the routers firewall, is to setup the IP address of the PS4 in what's called the DMZ. Simply this just lets any and all internet traffic communicate with the PS4.
The more complicated but safer alternative is to forward ports like you alluded to earlier. Here is a list and type of the ports you will need to forward. Once this is done correctly, your NAT type should change.
This can all be done from within the routers configuration pages.
