There is an RFID chip in the LTO cartridge that identifies the tape. Typically, barcodes are used by tape drives and silos to ID a tape.

I have the same problem and I found this fix in a service manual. It worked, but my tape motor adjustment is off. So be carefull.
The 3v dc is easy to make, connect 2 batteries 1.5v in serial, you get 3V dc.

input the security code, if you losen your card, remove the audio panel and then pull out the radio and tape player partially, read the serial number shown on the name card of radio and tape player,look-up the anti-theft code corresponding to the serial number code table, or make inquire to mitsubishi service center

Honda outlines it here:

You can retrieve your 5-digit code if you have your VIN (look for the plate at bottom of windshield on drivers side, looking from the outside), your phone number (the one Honda has on file, gathered when you bought it from the dealer) and the radio serial number.

They give you instructions on getting the radio serial number from the radio display which did work. Turn on ignition, have radio off and while simultaneously holding the 1 and 6 radio buttons push the "on" knob. Two code will flash... a U code and a L code, each followed by 4 numbers. Those 8 numbers (without the U and L form the radio serial number
