I have an ADATA 32 GB USB 3 flash drive which until recently was working perfectly for backing up data. Now when I plug it in the light flashes continuously and the drive does not open or show in Explorer. I have tried it in several computers with the same result. AT first the problem started out as intermittent (more fails than starts) but now it ndoes not seem to matter what I do. As far as I can tell the terminals in the USB plug are clean and undamaged, and it has not been wet or suffered any mechanical damage. It is a robust rubber covered pen drive. I would like to at least recover the files of the drive and then try to reformat it for further use.
You'll need to try a file recovery program to attempt file recovery.... Bad Copy Pro 4.10.125 works great for me. Then you will need to reformat the card to fat32 file system. That software is usually available from the manufacturer. It sounds like you have a defective card but you can try the above to see what the results are.

SOURCE: pen drive problem
Vista doesnt recognize all pen drives. only few like Sandisk, Transcend etc get recognized. Even brands like kingston don't get recognized. so to make them work install the driver for the pendrive. If you specify the brand of your pen drive ..i will try to help out with a driver .

i) try to put the usb drive in some other ports(such as usb port present on the backside of yur computer tower) and check whether your computer is detecting the drive.....
ii) if not there is sureluy some problem with ur usb drive, replace it or repair it..., you can even try putting another usb drive(if you have an spare one...)
