Check if it has a scrambled image on all sources ie - DVD- VCR - cable/satellite box. Since the PIP is normal it sounds like the signal source is bad.

do you have any scrambled blocks with any other game? if not, try cleaning the CoD 4 Disk. wipe from the inside out with a lint-free cloth. do not use a circular motion.
Use the composite connections (Yellow, Red, White) instead of the HDMI.
If that does not work, return the disk to the store. If you cant, call Activision and tell them that you have a corrupted disk. they should give you instructions on how to send it in. I had a problem with the disk "dobj for xmodel ‘body-mp-usmc-woodland-assault’ has more than 128 bones (see console for details)". just called them and told me to send them the disk. 2 weeks later, got a ticket to get a new CoD 4 Disk from the store, free.
