This can be caused by a hardware error or a software(driver,Windows,Etc..) error, or because of a conflict with another piece of hardware if you are using windows 98 or newer, right click on "My Computer" left click on properties, and click on the "Hardware" Tab in the System Properties window, Then click on device manager, is the modem listed under the "Modems" category, if so double click on the line for the modem, and if there are problems with the device it may tell you and give an error code, if it is not listed, is there anything EXCEPT a PLUS SIGN, (Example, an exclation mark or an X) if so those pieces of "Hardware" may have driver problems, goto the manufacturers page and look for the driver in the support/downloads section, and choose your OS and try re-installing the driver, OR if a new driver has recently been Updated or installed, you may try to "roll Back Driver, by clicking the button after double clicking on the "device".
