Why is my ticket asking me to file a claim form

The claim form supplies information to the particular lottery which establishes your claim to a prize. It would be well to fill out such before proceeding to a location to claim the prize. 04098 - Special Policy Adjustment - Turn Signal/Hazard Warning Flashers

Models:

1999 Oldsmobile Cutlass
1999-2002 Pontiac Grand Am


Supercede:

This bulletin is being revised to include 2001-2002 Chevrolet Malibu, Oldsmobile Alero, and Pontiac Grand Am vehicles. Due to part availability, it is anticipated that customers will not be notified until late April 2006. In the meantime, should a customer bring in a 2001-2002 Chevrolet Malibu, Oldsmobile Alero, or Pontiac Grand Am vehicle because the turn signal/hazard warning flashers are inoperative, repair the vehicle as described in this bulletin and submit the claim using the labor code found in this bulletin.

Please discard all copies of bulletin 04098 issued December 2004.

Condition

Some customers of 1999-2002 Chevrolet Malibu, Oldsmobile Alero, Pontiac Grand Am, and 1999 Oldsmobile Cutlass may have turn signal/hazard flashers that become inoperative.

Special Policy Adjustment

This special policy covers the condition described above for a period of 10 years or 150,000 miles (240,000 km), whichever occurs first, from the date the vehicle was originally placed in service, regardless of ownership. The repairs will be made at no charge to the customer.

For 1999-2000 vehicles covered by Vehicle Service Contracts, all eligible claims with repair orders on or after December 23, 2004 are covered by this special policy and must be submitted using the labor operation codes provided with this bulletin. Claims with repair orders prior to December 23, 2004 must be submitted to the Service Contract provider.

For 2001-2002 vehicles covered by Vehicle Service Contracts, all eligible claims with repair orders on or after January 11, 2006 are covered by this special policy and must be submitted using the labor operation codes provided with this bulletin. Claims with repair orders prior to January 11, 2006 must be submiffed to the Service Contract provider.

How do I submit a claim?

Don't understand what you mean, "Submit a Claim"

Do you mean return item to the shop? If so, you may be out of luck. Small print DVD Games, with most sales is non returnable. But for customer care they might exchange, but don't hold you breath?

On a possible brighter note, before committing "Hairy Curry", are you able to try losding to another poota?

But first, try a slighty damp cloth and carefully wipe the playing surface, dispite what they would have you believe, a slight piece of grime/finger print/at the right place/ cancels setup/ It might just do the trick.

How do I know/ into buying box sets, every so often, in a series, there is usually one that won't play, so its wipe its little botty and it plays.

Kodak works on a different kind of warranty where you are able to book the camera in for a warranty repair and on the third time repair if it gets to that you can have the camera replaced. If your camera is still within the 28 day period of purchase, you are able to take it back to the store of purchase. Overheating - Cooling Fans Inoperative - Replace Module Prior To Retailing - Service Action S932
This Service Action Applies to Vehicles That Have Not Been Retailed
Issue:
The cooling fan module on X-TYPE vehicles may fail under certain circumstances. There are various root causes but the design and quality of the printed circuit board within the cooling fan module are the most significant. The NAND gate used in the circuit is detective causing multiple failure modes, this can result in high engine temperature and overheating due to the cooling fans not operating.
A change has been implemented in production from VIN C91720. On these vehicles a 'Philips' printed circuit board has been used that has a modified voltage spike protection strategy.
Action:
To prevent customers from experiencing this issue, install the latest specification cooling fan module on 2002-03 MY X-TYPE vehicles within the above VIN range that have not been retailed.
PROCEDURE
1. Install new cooling fan module (see Workshop Manual, JTIS CD ROM, section: 303-03, SRO 26.25.38).
Parts Information
Warranty Information:
Warranty claims should be submitted quoting S932 in place of the warranty fault code together with SRO: 26.25.38 and the relevant part number. This will result in a labor time allowance of 0.8 hours to replace the cooling fan module and payment for the part used. Should the vehicle be brought back into the workshop for this action alone to be undertaken, an additional labor time allowance of 0.1 hours can be claimed quoting SRO: 10.10.10 for drive in/drive out.
Warranty claims should be submitted quoting the information found in the table. This will result in payment of the stated time and, where applicable, parts/miscellaneous expense codes.
The options that allow for drive in/drive out may only be claimed when the vehicle has been brought in to the workshop for this action alone to be undertaken.
The SROs and parts items listed above have been included for information only, there will be no requirement to enter the parts or SROs onto the claim.
NOTE: Always perform a DDW claim search first to determine whether this service action has been performed on this vehicle. The warranty selection under the vehicle inquiry will give a listing of all claims against the vehicle. If this service action number appears in the program code field, do not perform this service action.

None functioning product

Submit Proof of Claims to the bankruptcy court. Form can be pulled from the website. That's what I did. Cannot turn on the unit.
