Company name is Huazheng Electronic, model is CH-4116D input is ac 220-240v 50/60hz.
I have exactly the same adapter and it literally burnt out and I mean burnt out. Luckily I was at home but if I hadn't been god knows what would have happened. I have seen a German site which states that this adapter should have been withdrawn...but admittedly this was the 2 pin version.. I seriously think you should watch yours very very carefully.

If it is charging properly I would not worry about it. Chargers are a device that ALWAYS make noises, there just often out of our range. If it is making a VERY loud noise, then I would suggest locating a replacement power supply.

if its not usb mic you will have the humming
if it is a usb mic the port that you connected to needs a mic to be further away from comp your picking up a static charge from comp to mic
