If this was a card provided for you by your cell provider then you may be able to get the code from then. or it may be on the box/paperwork that came with the phone.

Hey, have no worries. I've just unlocked my memory card.
First of all I have a new memory card that I've been using lately. When I replaced it with my first memory card it had a lock sign on the icon, and when pressing to access it , it said "enter password". This I never even had. After fidling about with it and getting a little frustrated, I had an idea. First of all, I switched memory cards, opened up the options menu, where it said "set password" then I set a password, for example "1234". It told me to re-enter my new password, "1234", then it said password set, then I swapped back over the locked memory card, and when I opened the memory card icon, it asked for my password, which I entered "1234", and hey presto, my memory card has now opened.
It worked for me , without me having to format my card, hey,If it works for you guys, then please do let me know. GOD LUCK!!!!
