You have to ring your provider ie carphone warehouse,o2 etc and they will ask you for the long sim number and they will give you a code... your phone is blocked until then

Just call your provider and tell them you need a PUK code, they will be more than happy to give it to you.
you may also get it from the company website or go into the provider store.
