There are two caps on the power supply board that frequently go bad. The bad cap will be slightly swelled up and when you remove it and the bottom will probably be blown out. The bad cap on my supply only had a slight dome at the top to show that it was bad. The caps APEX use are 25V 1000uf. They are located where the ribbon cable plugs into the board, surrounded by a a heat sink w/ an inductor in between them. I went to Radio Shack to get a replacement. They only had the 35V 1000uf but you can still use it. Just as long as it's above 25V.

I was having this exact problem. You have to go to the "Audio MIDI setup" icon in the utilities folder. plugin your mac to the HDMI and TV and it should show up on the menu bar on the left as "HDMI"- then click it. then you have to click on the "format" drop down menu and change it to either 48000.0Hz or less. Make sure that the volume is down on your TV bc I almost gave myself a heart attack when I clicked it. Apparently the TV HDMI input is not compatible with the default on the Mac. Hope this helps!
