WOW! what a process! I don't know how you kept track of all that but , good job! Try this. It's a maybe, but better than nothing,
Put a non-accepted SIM Card and turn it on. Type *#9998*3323#. Press Exit. You will get a new menu. Select Malloc Fail.

PUK Code is always available with the network service provider and that too with no extra cost.
Call your network service provider and they'll in-turn ask you some questions which,if you are the owner of the sim,will be able to answer (piece of cake).like what is your last billing amount? In whose name is the sim registered? and all that.
Keep a note of the PUK it is unique and will always remain the same for your SIM.
For your PIN code...it is 1234 by default n when it asks for Password then its 00000000 (8 times zero).

You can get the PUK code by contacting your current service provider customer care department.If your phone is locked you can use any of your friends phone to make this call.The service provider will then provide you the PUK codes.. Remember that the reason why the codes are provided only through customer service provider is they will make a check on your name and identity proof at the time of your call. If the information is found to be correct and matching with their data then you will be provided with the codes. Everything is done for free of cost and you are not going to be charged for this process.This is the universal solution and is useful for many customers who are facing these kind of problems. Don't believe in any websites which provides PUK codes. All are fake. Only your service provider can provide these codes.

There can be many ways to Unlock your phone. As far as i know unlocking by codes is the simple and safe way. I will suggest the website Unlock-Zone.com which is doing good service and they were recommended highly in the cell phone forums.
I used them to unlock my cell phone. contacted them and within 30 mins i had my unlock code and my phone was unlocked.
