Inadequate Hot Water

Possible Causes
•Unit is undersized for water heating demands
•Broken or damaged dip tube allowing cold and hot water to mix in tank
•Faulty plumbing installation has crossed cold and hot water connections
•Faulty electric lower heating element or high or low heating element thermostat

Possible Repairs
•Make sure water heater is not being overtaxed by hot water supply demands. The water heater should have 75% of its capacity as hot water (e.g., a 40 gallon WH should be used for a demand of 30 gallons). To determine required capacity see Calculating Fixture Flow Rates.
•Undo cold water inlet and pipe nipple and remove dip tube. Check condition and replace if required.
•Check for crossed connection by turning off water supply to water heater. Open hot water tap at a faucet. If there is water flow, then the a cross connection exists somewhere. Check for a hot water line connected to a cold water connection on the water heater or appliances such as washer, dishwasher, faucet or shower valves.
•Check for power and electrical continuity at the lower heating element. Replace if needed but clear tank of any sediment first.
•If element tests OK, check for power at upper electrical upper thermostat. If OK check lower thermostat. Replace if necessary.

Not Enough Hot Water:

Thermostat set too low - Increase thermostat setting not to exceed 120 degrees
Defective lower element - Check and replace
Defective lower thermostat or miswired thermostat - Check and replace
Scale formation on heating element Check elements - clean or replace
Poor grounding of tank - Check grounding and tighten
Heater is undersized - Resize residence and compare
Damaged dip tube - Check and replace

In your situation - it is most likely a thermostat or element has failed.

5 years is about the lifespan for tankless.
You might have shorted element. That would explain cooler water and tripping reset.
Test element: power is off, disconnect wires from element; ohms = voltage squared divided by watt rating of element

Rinnai RL94i - Produces hot water for 3 minutes, then stops

I just had this exact problem. It kept getting worse until we had no water. Our Rinnai tankless hot water heater started the same way though, with hot water for a few minutes and then would shut off with an error code 10. We just had to shut the hot water off and then back on, wait a minute or so and we had hot water back for a few minutes. Our unit would come on with a loud vibrating noise and remain until it shut off with an error code 10 (air flow fault). The solution was to remove the blower fan and blow out the squirrel cage. Ours had moths in it. We also vacuumed out the fan housing. Be careful to mark any wires you disconnect and take pictures before removal. Our unit is now almost silent when it comes on and supplies endless hot water.

No hot water from kitchen tap

The hot side of the faucet is most likely plugged with something that came up thru the water system. The average thankless waterheater requires at least 1/2 to 3/4 gpm flow rate for the unit to sense flow and turn on. If there is not enough flow it will not fire and heat the water. Not sure about this particular unit but a few require at least one full gallon per minute flow. So chances are that if you fix the faucet, you fix the hot water issue at the same time

The thermal cutout, on the left side, pops up when water stops flowing through the unit. The modules are Polymer. I can press the button, have tons of hot water as long as the water is flowing through...

The left canister probably has a grounded element in it, which will keep power going after the water is off. Either that or you have a bad triac on your control unit

The button could be stuck on the thermostat or needs replacing. You try to reset it by putting the screw driver head against the reset button and bump the handle with your hand to try to free it up. I have tried that and it free up one, reguardless if it didn't you would have to replace it any way if it is bad.
The button should pree in to reset, also check element's out they can short and throw your breaker.

Too many possibilities to solve without being there. If you have to reset it, that means that one of the many safety switches kicked in. Be careful that you don't make it a habit of resetting it without getting it checked, doing so will mostly likely cause a lot more problems.
