I -believe- the second island opens after the Phnom Penh '86 mission at Diaz's mansion (which is located on Star Island in the middle of the map). I usually save at the house across the street from the lawyer's office (Ken Rosenberg) before doing this mission. Especially if you're not very good at aiming without auto-target, like me. *laugh*
