If I clean up concrete contaminated by a post-1987 spill pursuant to state clean-up standards, can I continue to use the concrete? If not, what are the requirements?

You may use non-liquid materials such as concrete that were contaminated with PCBs $50 ppm provided the materials are decontaminated in accordance with a PCB disposal approval, or an applicable PCB spill cleanup policy, . The decontamination standard for concrete is <10 µg/100 cm² and requires that cleanup be initiated within 72 hours of the spill. If the state cleanup met these standards, you may continue to use the concrete. Skype : wang.michelle35

I think you were told that by someone who was taking into consideration the room needed to drain the spa and the clearance needed for the drain plumbing.
But you can rest the spa on the ground but I would at least put down some type of barrier between the fiberglass and concrete.
Fiberglass is very soft compared to concrete

no, they are designed to require no support. You may use small columns of expansion foam for added strength, but do not fill entire space with foam as it will raise the whole floor upon expansion.

I am instaling a shower under my house and i was hoping to get away with not cutting into the concrete for the drainage, is it possible to raise the shower bottom a foot of the ground for the drainage, and...

Yes, you can raise to avoid the cut by raising shower as long as gravity drain can occur, i.e. you can drain the shower to connect to existing drain lines. Definately put the s-trap in, if no trap then plumbing gas comes back and shower will smell. Remember to put in a plumbing vent or tie in to existing vent.
