mfr., model of fireplace? you have to buy it from the fp mfr. or its retailer , i suppose.

Plug something else in to same receptacle to test if power at receptacle. You may have tripped breaker in panel that powers that receptacle

The fak 1500 number is for the blower assembly. I found this manual for a fireplace using the blower assembly. hope it is what you are looking for. Lennox has bought out the fireplace company and there are no manuals available under superior.

it has a fan as an option safe to use without! fan are located on very back if you look in bottom door, if it is an insert the fan may be located behind a panel behind the logs. if vented through a wall fan is underneath and has a heat sensor and comes on after fp runs for 10 minutes

verify the breaker is on (reset) then power up and verify voltage with a meter, there are safty devices that will keep the unit from energizing other than the switch

The blower is controlled by a thermostat and will only come on when it's hot. If still does not come on check thermostat.
