Finding a PUK (Personal Unblocking Key) number is not tough task. To find a PUK code all you have to do is call up customer care and ask them to help you with the issue. Before they provide you the PUK code, they will ask you several questions. You need to give them the appropriate information. Customer care are the only source from which can give you the PUK code.
At the time when you enter a wrong pin number three times. The phone asks you for a PUK number. The PUK code is generally eight digit number This PUK number is an unlocking code which helps you to unlock the cell phone and use it. If you enter a wrong PUK number 10 times than you simcard gets rejected. Then you will have to get a new simcard. This might happen that your phone might ask you for PUK2 after you enter your PUK number.

No the Default PUK and PIN codes are not set by you, these codes are Service Provider Dependent (SIM)
All you have to so is to call your Service Providers Customer Support (Like of T-Mobile, AT&T...).
And ask them about the PUK and PIN code, they will first Authenticate few things about the Ownership of the SIM card, and will give you the codes, in return.

PUK code can be given only by your service provider which is specific for each and every mobile number.
Contact your sim customer care department over phone. Its free of cost to get the pin or puk no. If you cant contact through your phone get some other phone and contact them.
Go to the nearest service outlet of your service provider they can also help you.
There is no other go than the above procedure only your service provider cangive it nobody else in this world nor any software.

Every PUK code is unique so nobody can tell you your PUK number, but i will tell you what you can do!
1) If you still have your packet that your Sim Card came in, you should find your PUK number in there, if you don't have the packet, please read bellow.
2) You must get a phone that is on the same network as your phone and call the networks Customer Care and ask them for your PUK number, they will ask you for your phone number, then they will tell you your PUK number. Once you have done this, you will be able to reset your Pin number so you will be able to go into your phone again.
