PUK is unique to each mobile network provider. This means you need to get it from your mobile phone provider or there may be an option to retrieve it on their web site (rare but some providers have this)
With your sim came a pin code (you could change that) and a PUK. After you entered a wrong pin in 3 times, the phone is locked. You than use the PUK to reset the PIN.
The puk cam on the came card or paper you initial pin came. (often the card you broke out the SIM)
