You should create a simple spreadsheet form. There are several programs out there that will work for you. I would suggest that Excel be used.

Personally I consider it a very bad idea as you would loose control of the expenses and quickly find your self in unpayable debt as all debt would be on your card and account
Let the family members apply for cards in their own names so that they have to take responsibility for their expenses
